Job summary

CenterWell Home Health is hiring a Speech-Language Pathologist to deliver in-home therapy and collaborate with the Clinical Manager to support patients in regaining communication and swallowing skills, independence, and quality of life.

Evaluate patients at home, develop and review plans of care, and provide training to patients and families. A license, CPR certification, and reliable transportation are required.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiology screening.
• Observe, record and report changes in the patient's condition to supervisor or physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation; prepare and submit reports as required by policy.
• Participate in care coordination and discharge planning.
