Job summary

CenterWell Home Health is seeking a Speech-Language Pathologist to evaluate and provide speech, language, voice and swallowing therapy in patients' homes or facilities. You will participate in treatment planning and care coordination, monitor progress, and document outcomes.

You will assess treatment effectiveness, educate patients and families, and maintain high professional standards and confidentiality throughout the care process. A valid license, CPR, and reliable transportation are required.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
• Observe, record and report changes in patient's condition and response to treatment to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility; prepare written reports.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality of patient information in accordance with policies.
