CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home or facility-based therapy focused on rehabilitation and independence. You will evaluate, treat, and document progress, while collaborating with a care team to optimize plans of treatment and discharge planning.

The role involves direct patient care, counseling families, and adherence to confidentiality and policy requirements. A valid license, ASHA credentialing, CPR, and driver’s license are required.

Qualifikationen

• Certification by ASHA for SLP licensure and credentialing.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license to practice.
• CPR certification current and active.
• A valid driver’s license, auto insurance and reliable transportation.

Aufgaben

• Evaluate, direct and provide speech-language pathology services in home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapies including audiology screening.
• Document changes and progress; report to supervisor and physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain required documentation and prepare reports and care plans.
• Participate in care coordination and discharge planning.
