Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings, with a focus on restoring communication abilities and independence. You will contribute to treatment plans, document progress, and collaborate with care teams to ensure high-quality, patient-centered rehabilitation.

The role requires ASHA CCC eligibility or certification, a valid license, CPR certification, and a valid driver's license for travel to patient

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology (ASHA) or Audiology
• Minimum six months experience as a speech therapist/speech-language pathologist
• Current and unrestricted license
• Current CPR certification
• Valid driver's license, auto insurance, and reliable transportation

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in home or facility
• Participate in development and review of Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and applications of therapies
• Observe and report changes in patient condition to supervisor/physician
• Provide instruction in alternative communication systems when appropriate
• Counsel patients, families and healthcare staff
• Submit documentation and reports as required by policy
• Participate in care coordination and discharge planning
• Maintain confidentiality and ethical professional conduct
