Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings. You will participate in treatment planning, perform diagnostic and therapeutic activities, and document progress for ongoing care coordination.

Role requires ASHA CCC- SLP/CCC-A, at least six months of experience, a valid license and CPR certification, plus a valid driver’s license and reliable transportation. Home health experience is preferred.

Qualifications

• Must hold Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA).
• Minimum six months of experience as a speech-language pathologist.
• Current and unrestricted professional license.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Use professional skills to assess and treat speech, voice, language, hearing and swallowing disorders to prevent, identify, evaluate and minimize effects.
• Administer and interpret diagnostic tests and therapeutic treatments including audiology screening.
• Observe, record and report changes in the patient’s condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel and instruct patients, families and healthcare staff.
• Maintain and submit required documentation including evaluations, visits, care plans and progress reports.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and share information only as rightfully required.
