Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver at-home or facility-based speech-language pathology services, focusing on rehabilitation and functional communication goals. Requirements include ASHA CCC-SLP, a current license, CPR certification, and a valid driver's license. The role involves patient-facing duties with TB screening and a driver-safety program.

Qualifications

• ASHA CCC-SLP Certification required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapy applications.
• Record changes and communicate with supervisor/physician.
• Instruct patients in alternative communication methods.
• Provide counseling to patients and families; coordinate with staff.
• Prepare written reports and care plans as required.
• Participate in care coordination and discharge planning.
