Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients at home or in facilities, under the Clinical Manager's guidance.

Responsibilities include developing treatment plans, documenting progress, and participating in care coordination and discharge planning. Requires ASHA CCC, current licensure, six months’ experience, CPR, and a valid driver’s license.

Qualifications

• Must have CCC from ASHA and current licensure.
• Minimum six months of speech-language pathology experience.
• Current CPR certification required.
• Valid driver’s license and transportation required.

Responsibilities

• Evaluate, direct and provide SLP services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat speech, voice, language, hearing and swallowing disorders.
• Administer and interpret diagnostic tests and screenings.
• Document evaluations, visits and care plans per policy.
• Assist with care coordination and discharge planning.
