Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver home-based therapy focused on rehabilitation and functional improvement. You will evaluate patients, develop care plans, and coordinate care with the clinical team to restore independence and enhance quality of life.

The role requires ASHA CCC or equivalent, at least six months of SLP experience, a current license and CPR certification, and a valid driver's license for travel to patients' homes.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology (ASHA CCC) or Audiology.
• Minimum six months of experience as a speech-language pathologist.
• Current and unrestricted professional license.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services in the home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing, and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families, and healthcare staff; maintain documentation.
• Participate in care coordination activities and discharge planning.
