Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver home-based SLP services, evaluate patients, and contribute to plans of care. Expect to work in patients' homes with emphasis on rehabilitation and functional improvement.

The role requires CCC-SLP certification (ASHA), a current license, CPR certification, and reliable transportation. Experience in home health is preferred.

Qualifications

• Must have CCC-SLP certification or ASHA eligibility.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license; CPR certification; valid driver's license and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Participate in Plan of Treatment/Plan of Care development and reviews.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; administer diagnostic tests.
