Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home or facility-based assessments and therapy. You will evaluate, treat, and document outcomes while coordinating with care teams to support patient rehabilitation and independence.

The role requires ASHA CCC credentials, at least six months of experience, a current license and CPR certification. A valid driver's license and reliable transportation are also required.

Qualifications

• CCC-SLP/CCC-A certification required by ASHA.
• Minimum six months experience as a speech-language pathologist.
• Current, unrestricted licensure and CPR certification.
• Valid driver's license with reliable transportation.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in home or facility.
• Participate in development and periodic review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapy applications, including audiologic screening.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; document evaluations and progress reports.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and appropriate information sharing.
