Job summary

CenterWell Home Health seeks a Speech Language Pathologist to deliver therapy in patients' homes or facilities, reporting to the Clinical Manager. You will evaluate, direct and provide speech-language pathology services to help patients regain communication and swallowing abilities, and participate in treatment planning.

The role requires developing care plans, documenting progress, and coordinating with families and healthcare staff to support rehabilitation and independence.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license; CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; prevent, identify, evaluate and minimize effects.
