Enhabit Home Health & Hospice is seeking a scheduling coordinator to arrange patient visits with field clinicians and ensure continuous, quality home-based care in South Carolina. You will liaise with patients, physicians, caregivers, and internal teams to maintain seamless coordination and timely updates.

The role requires an LPN/LVN license, CPR certification, a high school diploma or equivalent, and reliable transportation for on-site work.

Qualifikationen

• Minimum one year of clinical experience in a healthcare setting.
• Must hold active LPN/LVN license and CPR certification.
• High School Diploma or equivalent.
• Valid state driver license and automobile liability insurance.
• Proficient with basic technology and MS Word/Excel.

Aufgaben

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
