Enhabit, Inc. is seeking a Nurse Care Coordinator—Scheduling Specialist to coordinate patient appointments with field clinicians and ensure seamless care across teams.

You will facilitate communication among patients, caregivers, physicians and staff, while maintaining on-call and hospitalization records to support reliable care delivery. Required are an LPN license, CPR certification, English fluency, a valid driver's license, and strong technology skills.

Qualifications

• High school diploma or equivalent is required.
• LPN license and CPR certification are required.
• Valid state driver license and automobile liability insurance are required.
• Experience in home health, hospice or pediatric care within the last 24 months is preferred.

Responsabilités

• Schedule patients to field clinicians and coordinate care.
• Communicate with staff, patients, physicians and caregivers to maintain continuity of care.
• Manage on-call notes and hospitalization logs to improve stakeholder communication.
