Job summary

Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to arrange patient appointments with field clinicians and ensure seamless care coordination across teams. The role emphasizes strong communication with patients, staff, physicians, and caregivers to support high-quality home-based care.

Ideal candidates will hold an LPN license, CPR certification, and at least 1 year of healthcare experience, with familiarity in scheduling and use of Homecare Homebase (HCHB) preferred.

Qualifications

• High School Diploma or equivalent required.
• Licensed practical or vocational nurse required.
• Valid clinical license per state rules.
• Minimum of 1 year of clinical healthcare experience.
• CPR certification required.
• 1+ year of home health, hospice, or pediatric experience in last 24 months with staffing/scheduling exposure.
• Valid state driver license required.
• Automobile liability insurance required.
• Basic technology skills including mobile device use.
• Preferred experience with Homecare Homebase (HCHB).
• Preferred bilingual (English/Spanish) for clear communication.

Responsibilities

• Schedule patients to field clinicians.
• Coordinate with field staff, patients, physicians, referrals, and caregivers to maintain care coordination.
• Manage on-call notebook and hospitalization logs to improve communications.
