Job summary
Tufts Medicine Care at Home is seeking an experienced Home Health Registered Nurse to deliver professional nursing care in patients' residences. You will assess needs, plan care, and coordinate with physicians and families to support safe, effective outcomes.
The role requires an active RN license, BLS, and experience in acute care. A BSN and specialty certifications are preferred, with robust benefits and opportunities for growth within the Tufts Medicine system.
Qualifications
- Active RN license in the state where services are provided with no licensure gaps.
- 1 year of acute medical/surgical nursing experience preferred.
- Current BLS certification required; CPR may be required per hire date.
- BSN degree preferred and specialty certifications desirable.
Responsibilities
- Assess patients’ physical and psychosocial needs using established nursing standards.
- Develop and implement individualized plans of care with the care team.
- Provide comprehensive home health nursing care and clinical support tasks.
- Coordinate with physicians, families, and community resources for discharge planning.
- Document patient data and communications accurately in the charting system.
- Administer medications, reconcile changes, and ensure safety in the home setting.
- Triage patient visits and manage responsibilities across evenings, nights, weekends, and holidays.
