Tufts Medicine Care at Home seeks a Home Health Registered Nurse to manage and deliver comprehensive home health services in patients' residences. You will perform assessments, interventions, and supportive care, collaborating closely with physicians and the care team to prevent hospitalizations.

Requires an active MA RN license, 1 year of acute nursing experience, BLS, and CPR per hire timing. This full-time position covers Malden and surrounding areas with competitive pay and day-one benefits.

Qualifications

• Active RN license in the state of practice with eligibility to work in MA.
• Valid state Driver's License.
• Minimum of 1 year of acute medical/surgical nursing experience.
• Basic Life Support Certification (BLS).
• CPR Certification required per hire timing.

Responsabilités

• Assess patients' physical and psychosocial needs and provide ongoing interdisciplinary evaluation.
• Plan and implement nursing care within the patient's plan of care and agency policies.
• Coordinate with family, physician, and care team for discharge planning.
• Document patient data timely and accurately in the charting system.
• Complete orders, levels of care, and OASIS documentation for assigned patients.
• Administer medications and reconcile with physician records.
• Maintain safety, proper body mechanics, and equipment use in home care.
• Provide culturally sensitive care and support to patients and families.
• Triages patient visits and messages by priority and urgency.
• Deliver evening, night, weekend, and holiday nursing coverage as required.
