Kona Medical Consulting and The Home Health Team is actively looking for a Home Health Field Registered Nurse (RN) in the Dallas-Fort Worth Metroplex. Qualifications

• Current RN license in Texas.
• At least 1 year of home health nursing experience preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation and valid driver’s license.
• Proficient with EMR/mobile charting.
• Professional, compassionate, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate effectively with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families
