Job summary

Kona Medical Consulting and The Home Health Team in Dallas‑Fort Worth seeks a dedicated Home Health Field Registered Nurse (RN) to deliver high‑quality in‑home nursing care. You will perform assessments and contribute to patient health goals with compassionate, evidence‑based practice.

You will develop care plans, collaborate with physicians and care teams, and ensure accurate documentation, all while promoting safety and comfort for patients in their homes.

Qualifications

• Current Texas RN license.
• 1+ year nursing experience (Home Health preferred).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
