Kona Medical Consulting and The Home Health Team is actively looking for a Home Health Field Registered Nurse (RN) in the Dallas-Fort Worth Metroplex.

The ideal candidate provides skilled nursing in patients' homes, performs assessments, creates care plans, and educates families while collaborating with physicians and care teams. Texas RN license and 1+ year home health experience are preferred. Competitive pay and flexible scheduling offered.

Qualifications

• Current Texas RN license.
• 1+ year of nursing experience (Home Health preferred).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver's license, and proof of auto insurance.
• Compassionate, professional and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation per regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
