Kona Medical Consulting and The Home Health Team are actively seeking a Home Health Field Registered Nurse (RN) to provide skilled nursing care in patients' homes across the Dallas–Fort Worth area.

The ideal candidate is reliable, organized, and confident in clinical decision-making, with a passion for helping patients achieve health goals and maintaining compassionate, patient-centered care.

Qualifications

• Current RN license in Texas is required.
• 1+ year nursing experience; home health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate with physicians, case managers and interdisciplinary teams.
• Complete accurate, timely documentation per regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
