Kona Medical Consulting is seeking a dedicated Home Health Field Registered Nurse (RN) to provide skilled nursing care in patients' homes across the Dallas–Fort Worth Metroplex. The role emphasizes reliable clinical decision-making, compassionate patient care, and partnership with physicians and care teams.

The ideal candidate will hold a Texas RN license, have 1+ year in nursing (home health preferred), and be proficient with EMR/charting.

Qualifications

• Current Texas RN license is required.
• 1+ year nursing experience; home health preferred but not required.
• Strong clinical assessment and critical-thinking skills.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
