Job summary

Kona Medical Consulting and The Home Health Team are seeking a Home Health Field Registered Nurse (RN) in the Dallas–Fort Worth Metroplex. This role offers in-home skilled nursing visits and care planning by a patient‑focused team.

Ideal candidates have Texas RN licensure and at least 1 year of nursing experience, with EMR/mobile charting proficiency. Competitive pay, flexible scheduling, and growth opportunities are provided.

Qualifications

• Current RN license in Texas.
• 1+ year nursing experience (Home Health preferred).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation per regulatory requirements.
• Participate in on-call rotation as needed and build trusting relationships with patients and families.
