Job summary

Community Health Network is seeking an experienced Home Health RN Case Manager to support patients in the Norwood/Westwood, MA area and surrounding towns. You will manage a caseload from admission to discharge and supervise LPNs and Home Health Aides.

This role is Monday to Friday, 40 hours per week with no weekends or holidays, occasional on-call, and offers a highly competitive pay rate plus IRS mileage reimbursement along with strong benefits.

Qualifications

• Minimum 1 year of recent Home Health RN Case Management experience with OASIS visits for a Medicare certified agency.

Responsibilities

• Manages a caseload of patients and families in the community from admission to discharge.
• Supervises LPNs and Home Health Aides.

Well established home care agency has a need for an experienced Home Health RN Case Manager to see patients in homes in a coverage territory to include the Norwood/Westwood, MA area and surrounding towns.

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.




Requires minimum 1 year of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
