Job summary

Community Health Network, Inc. is seeking an experienced Home Care RN Case Manager to oversee patient care in the Hopkinton/Southboro/Framingham/Ashland MA area.

You will manage a caseload from admission to discharge and supervise LPNs and Home Health Aides across community settings. This full-time role offers mileage reimbursement and benefits including health, dental, vision, 401(k), life insurance, and access to a large library of free online CEUs.

Qualifications

• Minimum 1 year of recent Home Care RN Case Management experience with a Medicare certified agency.
• Experience performing OASIS visits.
• Active RN license in Massachusetts.

Responsibilities

• Manage a caseload of patients and families in the community from admission to discharge.
• Supervise LPNs and Home Health Aides.
• Coordinate care and ensure high-quality home-based services.

Community Health Network has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests.




Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager to see patients in homes in the Hopkinton/Southboro/Framingham/Ashland MA area.

• Full-time, 40 hours/week, Monday-Friday
• Occasional weekend rotation may be required
• No on call
• Annual Salary Range $125,000 - $145,000 (based on experience)
• Mileage reimbursement

Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.




The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
