Job summary

Community Health Network, Inc. seeks an experienced Home Care RN Case Manager to join our team serving the Douglas/Webster/Sutton MA area. The role is full‑time, 40 hours/week, with no weekends, holidays, or on‑call.

Qualifications

• Minimum 1 year of recent Home Care RN Case Management experience performing OASIS visits.
• Experience supervising LPNs and Home Health Aides is preferred.
• Licensed RN in Massachusetts is required.

Responsibilities

• Manage and care for a patient caseload from admission to discharge.
• Supervise LPNs and Home Health Aides on the caseload.
• Coordinate care with families and other care team members.

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager.




Coverage needs primarily in and around the Douglas/Webster/Sutton MA area.

• Monday-Friday, 40 hours/week
• Full-time, Contract
• Competitive pay rate plus mileage reimbursement
• No weekends, holidays or on-call required!

The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.




Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
