Job summary

Community Health Network is seeking an experienced Home Care RN Case Manager to oversee a caseload of patients in the Norwell, MA area. This full-time role involves coordinating admissions to discharge care and supervising LPNs and Home Health Aides to ensure high-quality home-based care.

Minimum 1 year of recent Home Care RN Case Management experience with OASIS visits to a Medicare-certified agency is required.

Community Health Network has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests.




Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager to see patients in homes in the Norwell, MA area.

• Full-time, Monday-Friday, 40 hours/week
• Shorter work weeks considered

The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.




Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
