Job summary

Community Health Network, Inc. is seeking a Home Care RN Case Manager to serve patients in the Chelsea/East Boston/Winthrop, MA area. You will oversee a caseload from admission to discharge and supervise LPNs and Home Health Aides while providing high-quality home-based care.

The role requires at least 1 year of recent Home Care RN Case Management experience performing OASIS visits for a Medicare-certified agency, with a strong focus on patient and family outcomes and flexible scheduling options.

Responsibilities

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager to see patients in homes in the Chelsea/East Boston/Winthrop, MA area.

• Full-time, 40 hours/week, Monday-Friday
• Occasional weekend rotation may be required
• No on call
• Competitive pay with mileage reimbursement

Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.




The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
