Job summary

Community Health Network, Inc. seeks a Home Care RN Case Manager to serve patients in the Amherst/Hadley/Hatfield/Williamsburg, MA area. The role is full-time (40 hours/week) with occasional weekend rotation and mileage reimbursement.

Requires at least 1 year of Home Care RN Case Management experience doing OASIS visits for a Medicare certified agency. The RN will manage a caseload in the community, supervising LPNs and Home Health Aides, and ensuring smooth admission through discharge.

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager to see patients in homes in the Amherst/Hadley/Hatfield/Williamsburg, MA area.

• Full-time, 40 hours/week, Monday-Friday
• Occasional weekend rotation may be required
• No on call
• Competitive pay with mileage reimbursement

Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.




The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
