Job summary

Community Health Network in Brockton, MA is seeking an experienced Home Health RN Case Manager to serve patients at home across Brockton and surrounding areas. This full-time, Monday–Friday role features 40 hours weekly with no weekends, on-call, or holidays.

You will supervise LPNs and Home Health Aides, manage a patient caseload from admission to discharge, and perform OASIS visits for a Medicare-certified agency; benefits include health/dental/vision, 401(k), life insurance, and CEU access.

Qualifications

• Minimum 1 year of recent Home Health RN Case Management experience with OASIS visits.
• Experience with a Medicare certified agency.
• Supervises LPNs and Home Health Aides.

Well established home health agency has a need for an experienced Home Health RN Case Manager to see patients in homes with coverage territory in and around the Brockton, MA area.

• Monday-Friday, 40 hours/week
• Full-time, Contract
• No weekends/on-call/holidays required!
• Highly competitive pay rate plus IRS mileage reimbursement

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.




Requires minimum 1 year of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
