Job summary

Community Health Network is seeking an experienced Home Health RN Case Manager to join our field team in the MA area surrounding Avon, Braintree, Canton, Holbrook, Mattapan, Milton, Quincy, Randolph, Stoughton.

You will manage a community-based caseload from admission to discharge, supervise LPNs and Home Health Aides, and coordinate care with patients and families. Candidates need at least 1 year in Home Care RN Case Management and an active RN license.

Qualifications

• Minimum 1 year of Home Care RN Case Management experience with OASIS visits for a Medicare-certified agency.
• Active RN license in good standing required.
• Strong ability to manage a caseload from admission to discharge.

Responsibilities

• Manage a caseload of patients and families in the community from admission to discharge.
• Supervise LPNs and Home Health Aides to ensure high-quality care.

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.




Coverage Territory to include in and around the Avon, Braintree, Canton, Holbrook, Mattapan, Milton, Quincy, Randolph, Stoughton, MA area.

• Monday-Friday, 40 hours/week
• Full-time, Contract
• Every 6th weekend rotation
• Highly competitive pay rates with IRS mileage reimbursement

The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.




Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare Certified agency.
