Job summary

Community Health Network is seeking an experienced Home Health RN Case Manager to support patients in the Beverly, Marblehead, Salem and Swampscott, MA area. This full-time role offers 40 hours per week, with potential weekend coverage and 13-week contracts.

You will manage a caseload from admission to discharge, supervise LPNs and Home Health Aides, and require at least 1 year of home care RN case management with OASIS visits for a Medicare certified agency.

Qualifications

• Minimum 1 year of Home Care RN Case Management experience.
• Experience performing OASIS visits for a Medicare certified agency.
• Strong ability to manage a caseload and supervise staff.

Responsibilities

• Manage a patient caseload from admission to discharge in the community.
• Supervise LPNs and Home Health Aides to ensure quality care.
• Coordinate care across disciplines and agencies as needed.

Community Health Network has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests.




Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.




Coverage needs primarily in and around the Beverly/Marblehead/Salem/Swampscott, MA area.

• Full-time, Monday-Friday, 40 hours/week
• Occasional weekends may be required
• Highly competitive pay rates with IRS mileage reimbursement
• 13-weeks contracts

The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.




Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
