Job summary

Community Health Network seeks a Home Care RN Case Manager to oversee patient care in the Quincy/Milton, MA area. This full-time role involves managing a caseload from admission to discharge and supervising LPNs and Home Health Aides in home settings.

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager to see patients in homes in the Quincy/Milton, MA area.

• Full-time, Monday-Friday, 40 hours/week
• Shorter work weeks considered

The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.




Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
