Job summary

Community Health Network, Inc. seeks a Home Health RN Case Manager to oversee patient care in the community across NH and MA. The role covers admission through discharge, coordinating with families and a care team.

The ideal candidate has at least 2 years in home health case management, performs OASIS visits, and holds active RN licenses in Massachusetts and New Hampshire. Working hours are Monday to Friday with 40 hours per week in a contract capacity.

Qualifications

• Minimum 2 years of recent Home Health RN Case Management experience.
• Experience performing OASIS visits with a Medicare certified agency.
• Must hold both MA and NH RN licenses.

Responsibilities

• Manage and care for a caseload of patients and families in the community from admission to discharge.
• Supervise LPNs and Home Health Aides.

Community Health Network (CHN) has been placing experienced nurses and physical therapists in home health, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests.

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.

Coverage territory needs include the Salem, Windham and Atkinson NH areas as well as into the Methuen to Haverhill MA area. (Must be both MA and NH RN licensed)




Monday-Friday (40 hours/week)

No weekends or holidays!

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.




Requires minimum 2 years of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
