Job summary

Qualifications

• Current license to practice as a physical therapy assistant (state-specific).
• Graduate from CAPTE-accredited PTA program.
• One year of related PTA experience in home health care; if less, regional mentorship approval.
• Current CPR certification.

Responsibilities

• Provides PTA services under supervision of licensed PT.
• Observes and reports patient response to treatment.
• Documents supervisory activities and patient progress.
• Coordinates follow-up care and referrals with the care team.
• Maintains patient confidentiality and quality improvement activities.
• Performs other duties as assigned.
