Job summary

UnitedHealth Group is seeking a Physical Therapy Assistant to provide skilled home health care under a licensed Physical Therapist. You will implement individualized treatment plans to improve mobility, strength, balance, and independence while ensuring patient safety and quality of life at home.

Responsibilities include documenting progress, coordinating with caregivers and the care team, and supporting care transitions.

Qualifications

• Graduate from CAPTE-accredited PTA program.
• Current license to practice as a PTA in the state of assignment.
• Current CPR certification.
• 1+ years PTA experience in home health or equivalent.

Responsibilities

• Provide physical therapy services under the supervision of a licensed PT.
• Record and report patient progress and changes to the supervising PT and care team.
• Support wellness and prevention; document supervisory activities in medical records.
• Coordinate follow-up care with patient, caregivers and referral sources.
• Participate in care conferences, chart audits, and quality improvement activities.
• Maintain confidentiality of patient information.
• Perform additional duties as assigned.
