UnitedHealth Group seeks a Physical Therapist Assistant (PTA) to provide skilled home health care under licensed PT supervision. You will implement individualized plans to improve mobility, strength, balance, and safety in patients' homes, coordinating with patients, caregivers, and the care team.

Responsibilities include documenting progress, supporting wellness, and contributing to quality improvement initiatives while adhering to regulatory and privacy requirements.

Qualifications

• Current PTA license in the state of assignment.
• Graduate from an CAPTE-accredited PTA program.
• One year related PTA experience in home health care, or regional leadership approval if less than one year.
• Current CPR certification.

Responsabilités

• Provides physical therapy services under the direction and supervision of a licensed physical therapist.
• Observes, records and reports patient response to treatment and any change in condition.
• Provides resources for future needs and promotes wellness and prevention.
• Includes documentation of supervisory activities in the patient’s medical record.
• Monitors and documents patient response to physical therapy intervention.
• Participates in patient care conferences, in-services, chart audits and quality improvement activities.
• Maintains confidentiality of information relating to the therapist-client relationship.
• Makes follow-up care recommendations coordinating with the admitting agency and other care providers.
• Performs other duties as assigned.
