Job summary

UnitedHealth Group is seeking a Physical Therapist Assistant to provide skilled care in patients' homes under a supervising Physical Therapist. You will implement treatment plans to improve mobility and independence, coordinate with caregivers and the care team, and document progress in the medical record.

Qualifications

• Current license to practice as a physical therapy assistant in the assigned state.
• Graduate from a CAPTE-approved PTA program.
• At least one year of PTA experience in home health care or similar setting.
• If under 1 year, regional clinical leaders' approval and mentor/residency participation is required.
• Current CPR certification.

Responsibilities

• Provides physical therapy services under the supervision of a licensed PT and plan of care.
• Observes and reports patient response and changes to the supervising PT and clinical staff.
• Promotes wellness, prevention, and coordinates with caregivers.
• Documents supervisory activities in the medical record.
• Monitors patient progress and participates in quality improvement activities.
• Maintains patient confidentiality and coordinates follow-up care.
