Responsibilities

• Provide PT services under supervision of a licensed PT per plan of care.
• Observe and report patient response and changes to the supervising PT.
• Offer resources and promote wellness and prevention for future needs.
• Document supervisory activities in the medical record.
• Monitor patient response and participate in care conferences and quality activities.
• Maintain confidentiality and coordinate follow-up with involved agencies.
