Job summary

UnitedHealth Group is seeking a licensed Physical Therapist Assistant to deliver in-home physical therapy under supervision of a Physical Therapist. You will implement individualized treatment plans to improve mobility, strength, balance and independence for patients in their homes.

Work with patients, families and the care team to support recovery, promote safety, and document care while adhering to privacy and quality standards.

Qualifications

• Current license to practice as a physical therapy assistant in the assigned state.
• Graduate from an accredited PTA program per CAPTE.
• One year of related PTA experience in home health or willingness to be mentored.
• Current CPR certification and valid driver's license.

Responsibilities

• Provides PT services under the supervision of a licensed PT.
• Observes, records and reports patient response to treatment.
• Promotes wellness and coordinates follow-up care with care providers.
• Documents supervisory activities in the medical record.
• Monitors and documents patient progress and participates in care conferences.
• Maintains confidentiality of patient information.
• Recommends follow-up care and coordinates referrals.
• Performs other duties as assigned.
