Job summary

Amedisys is seeking a Physical Therapist Assistant to provide skilled PT services in patients' homes under the supervision of a licensed Physical Therapist. You will implement individualized treatment plans to improve mobility, strength, balance and independence while maintaining safety in the home setting.

Responsibilities include documenting progress, coordinating with patients, caregivers, and the care team, and supporting recovery and quality of life at home.

Qualifications

• Current licensure to practice as a PTA for the state of assignment.
• Graduated from a CAPTE-accredited PTA program.
• One year of PTA experience in home health care recommended for entry.
• If less than one year, approval from regional clinical leaders and mentoring/residency involvement required.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide physical therapy services and interventions under the supervision of a licensed PT in patients' homes.
• Observe, document, and report patient responses to treatment and any changes in condition.
• Promote wellness and prevention and plan for ongoing care needs.
• Document supervisory activities in the patient’s medical record.
• Monitor patient responses to therapy and adjust plans as needed.
• Participate in care conferences, chart audits, and quality improvement activities.
• Keep patient information confidential and coordinate follow-up with involved agencies.
