Job summary

Primary responsibilities include delivering PT services under supervision, documenting progress, coordinating with the care team, and promoting wellness.

Qualifications

• Current license to practice as a physical therapy assistant in the assigned state
• Graduate from an accredited PTA program per CAPTE
• Current CPR certification
• 1+ year of PT experience for home health
• Driver's license and reliable transportation

Responsibilities

• Provides PT services under supervision and in accordance with the plan of care
• Observes and reports patient response to treatment
• Documents supervisory activities and patient progress
• Promotes wellness and prevention
• Coordinates follow-up care and referrals with the care team
• Maintains patient confidentiality
