Job summary

Amedisys is seeking a Physical Therapist Assistant to deliver skilled care in patients' homes under the supervision of a licensed Physical Therapist. You will follow individualized plans to improve mobility, strength, balance and independence while ensuring safety.

You will collaborate with patients, caregivers and the interdisciplinary team to support recovery and enhance home-based quality of life. A strong focus on documentation and care coordination is essential.

Qualifications

• Graduate from an accredited physical therapy assistant program (CAPTE).
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ year of PTA experience in home health or equivalent; if under 1 year, regional leaders' approval and mentoring may be required.
• Driver's license and reliable transportation.

Responsibilities

• Provide PT services under the direction of a supervising physical therapist.
• Document patient responses and any condition changes in the medical record.
• Promote wellness, prevention, and patient safety in the home setting.
• Coordinate care and referrals with the care team and community resources.
• Participate in care conferences, chart audits, and quality improvement activities.
