Amedisys, part of the Optum family, is seeking a Physical Therapist Assistant to deliver skilled home-based therapy under a licensed PT. You will implement individualized treatment plans to improve mobility, strength, balance, and independence, while ensuring safety at home and collaborating with patients, caregivers, and the care team.

You will document progress, participate in care conferences, and support referrals to community resources, with a focus on quality and compassionate care.

Formación

• Graduate from an accredited PTA educational program as set by CAPTE.
• Current license to practice as a PTA, specific to the state of assignment.
• Current CPR certification.
• 1+ year of PTA experience in home health or willingness to be mentored for less experience.

Responsabilidades

• Provides physical therapy services under the supervision of a licensed physical therapist.
• Observes and reports patient response to treatment and changes in condition.
• Documents supervisory activities in the patient’s medical record.
• Promotes wellness, prevention, and provides resources for future needs.
• Coordinates follow-up care with agencies and referral resources.
• Maintains confidentiality of patient information and participates in quality improvement activities.
