Job summary

Amedisys, part of the Optum family, seeks a Per Diem Physical Therapist Assistant (PTA) to deliver skilled care in patients' homes under a supervising PT. You will implement individualized treatment plans to improve mobility, strength, balance and safety, coordinating with patients, caregivers and the care team.

Qualifications

• Graduate from CAPTE-accredited PTA program.
• Current license to practice as a PTA.
• Current CPR certification.
• 1+ year PTA experience in home health preferred.
• If <1 year experience, mentoring/residency program required.
• Driver's license and reliable transportation.

Responsibilities

• Provides PT services under supervision of a licensed PT and per plan of care.
• Observes and reports patient response and progress.
• Documents supervisory activities in patient records.
• Participates in care conferences and quality improvement activities.
• Maintains patient confidentiality and coordinates with caregivers and other providers.
