Job summary

Amedisys, part of the Optum family, is hiring a Physical Therapy Assistant to deliver home health care under PT supervision. You will support patients at home, document responses, and coordinate care with the clinical team to promote wellness and safe discharge.

Requirements include CAPTE-accredited PTA program, current state license, CPR, 1+ year of home health experience or approval/mentoring if less, and valid driver’s license.

Qualifications

• Graduate from an CAPTE-accredited physical therapy assistant program.
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ year of related PTA experience for home health care or approval/mentoring if less than 1 year.
• Driver's license and reliable transportation.

Responsibilities

• Provides physical therapy services under the direction of a supervising PT.
• Observes, records, and reports patient responses to treatment.
• Promotes wellness and prevention through patient care.
• Documents supervisory activities in patient records.
• Monitors patient responses to interventions.
• Participates in care conferences and quality improvement activities.
• Maintains confidentiality of patient information.
• Coordinates follow-up care with admissions and other providers.
• Performs other duties as assigned.
