Qualifications

Responsibilities

• Provides physical therapy services and interventions under supervision per plan of care.
• Observes and reports patient responses and changes to supervising PT and team.
• Maintains documentation of supervisory activities and patient responses.
• Participates in care conferences, chart audits, and quality improvement activities.
• Keeps patient information confidential and coordinates follow-up care.
