Qualifications

• Current PTA license in the assigned state.
• Graduate of an accredited PTA program (CAPTE).
• At least 1 year of PTA experience in home health preferred.
• Current CPR certification.

Responsibilities

• Provides physical therapy services under supervision of a PT and according to the plan of care.
• Observes and reports patient response to treatment and any condition changes.
• Maintains documentation in the patient’s medical record.
• Participates in care conferences, chart audits, and quality improvement activities.
• Makes referrals and coordinates with other care providers for follow-up.
• Respects patient confidentiality and privacy.
