Amedisys Home Health is seeking a Physical Therapist Assistant to deliver high-quality PT care directly in patients’ homes under the supervision of a licensed PT. You will document progress, promote wellness, and collaborate with a supportive clinical team to improve mobility and independence for patients.

Responsibilities include implementing plan of care, monitoring responses, and coordinating with the agency for follow-up care. Equal opportunity employer with comprehensive benefits.

Qualifications

• Current license to practice as a physical therapy assistant in the assigned state.
• Graduate from an accredited PTA program as required by CAPTE.
• One year related PTA experience in home health recommended.
• If less than one year of experience, mentoring/residency may be required.
• Current CPR certification.

Responsabilités

• Provide PT services under the supervision of a licensed Physical Therapist.
• Observe and document patient response to treatment and any changes in condition.
• Promote wellness and preventive care.
• Document supervisory activities in patient records.
• Monitor patient progress and treatment response.
• Participate in care conferences, in-services, audits, and QA activities.
• Coordinate follow-up referrals with the admitting agency.
