Amedisys is seeking a Physical Therapy Assistant to provide in-home therapy services under the direction of a supervising physical therapist, delivering care per the approved plan of care.

The role requires a current PTA license for the assigned state, graduation from a CAPTE-accredited PTA program, CPR certification, and at least one year of home health experience. Mentoring may be required for those with less than one year of experience.

Qualifications

• Current license to practice as a physical therapy assistant in the assigned state.
• Graduate from a CAPTE-accredited PTA program.
• One year of related physical therapy experience in home health.
• Current CPR certification.

Responsabilités

• Provide physical therapy services under supervision and the physician-approved plan of care.
• Observe, record and report patient responses to treatment and any condition changes.
• Provide resources for future needs and promote wellness and prevention.
• Include documentation of supervisory activities in the patient’s medical record.
• Monitor and document patient response to PT intervention.
• Participate in patient care conferences, in services, chart audits and quality improvement activities.
• Maintain confidentiality of information related to the PT-client relationship.
• Make referrals to community resources and coordinate follow-up care with the admitting agency.
• Performs other duties as assigned.
