Job summary
Elite Home Health in Crystal River, FL is hiring a Licensed Physical Therapist Assistant (PTA) to provide skilled therapy in patients' homes under the supervision of a licensed PT. This PRN home health role requires an active Florida PTA license and CPR certification, serving patients in Crystal River and surrounding areas.
The PTA will support mobility, independence, and safe functional movement, documenting care and coordinating with the interdisciplinary team.
Qualifications
- Graduate of an accredited Physical Therapist Assistant program.
- Current Florida PTA license/certification in good standing.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Previous clinical experience preferred; home health experience is a plus.
Responsibilities
- Provide skilled PT treatments under supervision of the PT.
- Assist patients with therapeutic exercises, gait training, transfer training, and mobility.
- Support strengthening, fall prevention, and safe functional movement.
- Monitor patient response and report progress to the PT.
- Document accurately in the EMR and communicate with the care team.
- Participate in care coordination and required trainings.
- Follow Medicare/state regulations and agency policies.
